Submit Summaries for TIS Credit

READ CAREFULLY!  BE SURE TO USE THE CORRECT FORM BELOW!

Select your TIS "identification" from the list below.  You'll be linked to a Google form where you will answer a few questions about the session you attended.  As you'll see on the form, you can earn up to an additional hour for writing a detailed summary of the session.  If your summary isn't detailed enough (our judgement), your certificate will reflect that.  (SEE THE RED TEXT BELOW FOR MORE ABOUT THIS.)

The subtitle of this site is "resource site about webinars and online P.D.", so any of the topics listed on this site are pre-approved.  However, training tutorials that you watch to learn a tool or skill will probably not count towards these credit hours.  If you want one to be considered, please have it pre-approved. 

Every couple of weeks, I'll check the forms and send PDF certificates that you can send with your TIS application for authorization or renewal and insert in an e-Portfolio.  If you need a certificate sooner, email me.  bfulford@access.k12.wv.us 

THIS PAGE WILL BE REMOVED ON JUNE 30, 2013.  YOU WILL STILL BE ABLE TO SUBMIT SUMMARIES USING THE NEW FORMS ON THE TIS-SUMMARY FORMS PAGE.  IMPORTANT NOTE:  BEGINNING WITH THE NEW FORMS, WE WILL NO LONGER BE AWARDING EXTRA CREDIT FOR WRITING DETAILED SUMMARIES.
