Home-based record repository tracking sheet

An electronic and physical repository of home-based records is currently maintained by the WHO and UNICEF (see www.immunizationcards.org). 

The current page serves as a quick reference to identify countries currently not represented in the home-based record repository. Countries marked in red below indicate that a electronic / hard copy of the current vaccination card is not on file. Countries marked in green below, which indicates that both an electronic and a hard copy are on file, are encouraged to verify that the repository has the most up-to-date version of the home-based record used in your country. Any historical versions of the home-based records in a country may also be submitted and added to the library. If a country no longer utilizes home-based records to record primary health care services, including immunizations, please let us know that as well.

UNICEF and WHO headquarters, regional and country office staff are encouraged to facilitate the population of the library with current and prior versions of home-based vaccination records.  Hard copies of home-based records can be delivered to Ms Marta GACIC-DOBO, World Health Organization, Avenue Appia, 20, 1121 Geneva 27, Switzerland.
