Applications to the University of Kentucky's graduate programs in Clinical and Translational Science will be evaluated on the basis of content, clarity, fit with the program, and (if applicable) the identification of appropriate research mentors.

1. Before you begin your application, please review the Application Checklist for the program to which you are applying, so that you can most efficiently complete this application (downloadable versions are available below).  All materials in the Checklist, including Letters of Support, must be received by the Application Deadline in order for your application to be considered complete. The Application Deadline is May 1 for the Fall semester and October 1 for the Spring semester. Late or incomplete applications will not be reviewed until the following application cycle. 

Please note: If you are notified of your acceptance to the program, you will also need to submit an additional application to the Graduate School before you can matriculate into the program.

2. Please complete the application below to be considered for the University of Kentucky’s graduate programs in Clinical and Translational Science (CTS). This application will ask you to provide responses to several open-ended questions about your interest in Clinical and Translational Science; these items are included in the Checklist so that you can provide thoughtful responses.

3. You will be asked to upload electronic copies of your Curriculum Vitae (MS Word or PDF) and your transcript from your current or last attended institution (an unofficial copy is acceptable). A link will be provided at the conclusion of your application or you may click on the appropriate link below. Thank you.  

4. All applicants will need to request letters of support and ratings from individuals who are knowledgeable about you. Please refer to the program-specific Application Checklist for instructions about the individuals from whom letters are required. Click on the appropriate rating form below, and then send this link to the two referees who will be filling out your Rating Form and writing your letter of support.  This should be done well in advance of the Application Deadline (May 1 for Fall semester, October 1 for Spring semester). Rating Forms and letters of support are due along with all other application materials by the Application Deadline. Please note that all letters of support should be printed on institutional letterhead and must be signed by the referee before they are uploaded.
