Health and Fitness Questionnaire:


If you are between 15 and 69 years of age, the PAR-Q will tell you if you should check with your doctor before you significantly change your physical activity patterns. If you are over 69 years of age and are not used to being very active, check with your doctor.

Common sense is your best guide when answering these questions. Please read carefully and answer each one honestly by ticking YES or NO. 


  1. Has your doctor ever said you have a heart condition and that you should only do physical activity recommended by a doctor?
  1. Do you feel pain in your chest when you do physical activity?
  1. In the past month, have you had a chest pain when you were not doing physical activity?
  1. Do you lose your balance because of dizziness or do you ever lose consciousness?
  1. Do you have a bone or joint problem (e.g. back, knee or hip) that could be made worse by a change in your physical activity?
  1. Are you diabetic?
  1. Is your doctor currently prescribing medication for your blood pressure or heart condition?
  1. Are you pregnant or have you been pregnant in the last 6 months?
  1. Are you currently taking any medication? If yes, what and for what reason?

 

  1. Do you know of any other reason why you should not do physical activity?


Yes to one or more questions 1-4: You should consult with your doctor to clarify that it is safe for you to become physically active at this current time and in your current state of health. Yes to one or more questions 5-10: You can be reasonably sure that it is safe for you to participate in physical activity proved special consideration is given to the above, gradually building up from our current ability level. No to all questions: You can be reasonably sure that it is safe for you to participate in physical activity, gradually building up from your current ability level. By signing this PARQ document with agreeing that you have read, understood and accurately completed this questionnaire. And  confirm that you are voluntarily engaging in an acceptable level of exercise, and participation involves a risk of injury.

DH Fitness Ltd (previously Transformation Training) are NOT responsible for any financial or pecuniary loss caused as a results of any negligence or omission howsever caused.

It is you’re responsibility to notify us of any injuries or health conditions that arise after commensing a program with us.

By joining one of our programmes you are agreeing to let us know about any medical issues you have.




Definitions


1.1 ‘Transformation Training group programme is a 6month group programme that consists of 6 x months individual programmes. The number of sessions that a client receives per week for their monthly fee are three (3).

These all entail various sessions, but all include a nutrition programme, at home workouts, recipe booklet, essential shopping lists and much more.


1.2 ‘Fixed Term’ means 6 months


2. ‘One Off’ Membership


1. A One Off membership entitles the Transformation Training Member

membership to one Transformation Training group programme taking place on the dates , times and at the location specified at the time of registration. The number of sessions a client receives per week for their month fee are three (3)


3. 6 Month Membership

1. A 6 Month Membership entitles the Transformation Training Member

membership to each Transformation Training group programme which takes place during the term of the membership which is applicable to their class of membership

(e.g. South Shields  New Beginnings programme


4. Refunds and Cancellation

1. Once purchased, memberships are non-refundable and nontransferable.

2. A 6 Month Membership cannot be cancelled during the Fixed Term


3. On expiry of the Fixed Term a 6 Month Membership will

continue on a month by month basis until cancelled by giving

30days notice in writing.


5. Health and Safety

1. The Transformation Training Member shall notify their instructor if they suffer

from any health conditions or injuries.

2. The Transformation Training shall not be liable for any loss or injury attributable

to:

(i)the fault of a Transformation Training Member;

(ii) a third party unconnected with the provision of services provided

by The Fitness Camp

(iii) events which neither Transformation Training nor their suppliers or

agents could have foreseen or forestalled even if they had taken all reasonable care.


5. Money Back Guarantee

5.1 The 100% money back guarantee applies to ‘One Off’

Memberships only and the first month of the 6 month programme.


5.2 To qualify the Transformation Training group programme Members must have attended every exercise session in the specific programme they signed up for and provided their instructor with a completed food diary when asked for by a Transformation Training trainer.


5.3 If a Transformation Training Member qualifies for the money back guarantee

and is not satisfied with the results achieved they shall be entitled to a

full refund of the membership fees paid by them.


6. Intellectual Property


6.1 Any marketing, educational or other materials produced by Transformation Training Ltd and made available to Transformation Training Members will at all

times remain the property of the Transformation Training Ltd and will be subject to

copyright. Transformation Training members undertakes not to copy publish

or reproduce any such materials.
