We are a group of people who are dedicated to improving quality of life for persons at the end of life and their families. The Coalition was organized in September 2000, following the Bill Moyers production of Life on Your Own Terms. The coalition has grown and worked toward improving end of life care in the region by sponsoring various educational and community activities. Founding institutions were: Florida Gulf Coast University College of Health Professions, Hope Hospice and Palliative Care, Lee Memorial Health System, and Southwest Regional Medical Center.
All persons living in Southwest Florida will have equal access to the highest quality of health care at the end of life.
The Southwest Florida Palliative Care Coalition is a group of people dedicated to improving end-of-life care, from diagnosis to death, through research, education, and dialogue.
· In the interdisciplinary approach to compassionate end-of-life care.
· Individuals and families are an integral part of end-of-life care.
· Standards of care at the end-of-life are established through research and evidenced based practice.
· Knowledge regarding choices in end-of-life care is a fundamental right for all persons.
