Citywide Case Management

Citywide Case Management (CWCM), a UCSF/San Francisco General Hospital (SFGH) Department of Psychiatry program, has provided comprehensive and intensive outpatient Clinical Case Management services to individuals with serious mental illness since 1981. In addition, we have a forensic component for clients involved with the San Francisco legal system. Staff are devoted to stabilizing and improving clients' lives in the community.

Citywide Case Management has served as both a pioneer and prototype in San Francisco for treating individuals with serious and persistent mental illness. CWCM's clients are often perceived  as untreatable outside of a locked settings.  CWCM has enable clients to thrive in the community by providing the following services:
  • Intensive Case Management, featuring both routine and crisis community outreach
  • Carefully monitored psychiatric medication regimens
  • Long-term therapeutic relationships with Clinical Case Managers
