3. Two valid I.D. (with recent picture).
(Must have two clear photocopies)

4. Birth Certificate (if single) or Marriage Certificate (if married).
(Must have a photocopy.)

5.  Waiver of Conjugal and Affidavit (Must be notarized upon submission)
(If not naturalized Filipino)

6. Special Power of Attorney (SPA).
If principal buyer is not here or in abroad.Must be notarized upon submission.
