PTLD is a heterogeneous group of lymphoproliferative disorders that can involve different organs and can occur any time after transplantation. The manifestation of the disease and the anatomic pattern of organ involvement are highly dependent on the type of allograft. The imaging and clinical manifestations of PTLD are variable and are not predictive of histologic findings; thus, tissue sampling is always necessary. Familiarity with risk factors, the imaging appearance of lesions, and pattern of disease based on allograft type can allow a more accurate diagnosis of PTLD. FDG PET/CT is increasingly being used for the detection of extranodal involvement and for posttreatment evaluation.
