IMPORTANT: THIS APPOINTMENT SYSTEM IS ONLY FOR THOSE INTENDING TO APPLY FOR PASSPORTS AT THE PHILIPPINE CONSULATE GENERAL IN OSAKA, JAPAN. APPOINTMENTS MADE IN THIS PORTAL ARE NOT VALID IN ANY OTHER LOCATION.

Ang appointment system ay par lamang sa mga nagbabalak mag apply ng passport sa Phiippine Consulate General sa Osaka, Japan. Ang mga appointments na gagawin sa portal na ito ay hindi magagamit sa ibang lokasyon.

INSTRUCTIONS

STEP 1 : Check if there are still slots available for your preferred date.

Tingnan kung available ang inyong ninanais na petsa para sa inyong appointment.

STEP 2: Fill up the online passport application form. Make sure all the entries are correct and complete. Click next to go from one section to another. At the end of the form, click submit.

Sagutan ang online passport application form. Siguraduhing ang lahat ng inyong sagot ay tama at kumpleto. I-click ang submit.

STEP 3: If you are applying for passport renewal, email a copy of the data page of your current passport to passportcopy.osakapcg@dfa.gov.ph .

If a first time applicant or applying for a new passport, email a copy of the birth certificate of the applicant/child or his or her Report of Birth. Additional requirements apply for women who are changing their surnames as a result of marriage, annulment, foreign divorce or being widowed.

If the applicant is a minor (below 18 years old), you also need to email a copy of the data page of the passport of the parent.

Kung ikaw ay naga-apply para sa regular na passport renewal, mag-email ng kopya ng data page ng iyong kasalukuyang passport sa passportcopy.osakapcg@dfa.gov.ph. Kung menor de edad (below 18 years old) ang aplikante, kakailanganin din isama sa email ang kopya ng data page ng passport ng magulang ng bata. Kung first time o new passport applicant, i-email sa nasabing email address ang kopya ng birth certificate ng first time applicant/ng bata o ang kopya ng kanyang Report of Birth. May mga karagdagang requirements na kailangan para sa mga babaeng aplikante na nagnanais magpalit ng kanilang apelyido dala ng pagpapa-kasal, pagpapa-annul ng kasal, pagkaka-divorce abroad sa asawang banyaga o pagkaka-byuda sa asawa.

STEP 4: If your submissions are complete and the date you requested is still available, the Consulate will schedule your appointment and will send an appointment confirmation to the email address you provided.

Kung kumpleto ang inyong naisubmit na requirements at ang petcha na inyong ni-request ay siya pa ring available, padadalan kayo ng appointment confirmation doon sa email na inyong ibinigay sa inyong aplikasyon. Ang email na ito ay magkakalaman ng detalye hinggil sa oras at petcha ng inyong appointment, mga bagay na kailangan ninyong dalin sa Konsulado at mga importanteng paalala hinggil sa passport enrolment process.
