Things to do for your first appointment

Bring your ID (driver's license, passport, Nevada state ID, or military ID), insurance card, and patient's shot record.  If you have any old medical records, bring them with you.  Bring a written list of questions for the doctor (it's easier for you and us). 

Bring any medicines your child is taking.  If you try to write it down, you may not get the dose or strength correct, so just bring the actual medicine.  It makes it a lot easier for us.

Download the new patient paperwork at the following link.  Fill out the new patient paperwork before you arrive at our office and bring it with you to your first appointment.  Some parents find this much easier to do.  We have had some patients take 30 minutes filling out the paperwork at our office because they are busy feeding their newborn baby or trying to keep their two year old under control.  You can fax it to us at 733-7292 or e-mail it to us at, but I would also STRONGLY recommend that you bring a copy to your first appointment just in case the paperwork gets misplaced by us.
