Dear Professor Lesley Regan, We are writing in response to your college’s statement to the public consultation on the implementation of cell-free DNA testing (cfDNA), a form of prenatal testing (NIPT) for Down’s syndrome and other trisomies, into the national Fetal Anomaly Screening Programme (FASP). In particular, with reference to the comment that: “If the decision [not to offer the test to all women] has been made primarily on cost grounds, then a more rigorous economic analysis has to be made that includes the lifetime costs of caring for children and adults with Down’s syndrome… Such an economic analysis may (or may not) suggest that cfDNA testing for all is cost-effective”[1]. We are concerned that the position of the RCOG has moved from supporting women to make an informed choice, to advocating that women with a prenatal diagnosis of Down’s syndrome should end their pregnancy. It also alarms us that there is an assumption amongst the RCOG that a person with Down’s syndrome can not lead a happy and fulfilled life. This is not supported by the evidence into the contentment of people with Down’s syndrome and their families[2][3][4]. We utterly reject the implicit premise that the value of a human being is based on their economic contribution to society. Women deserve to be accurately told the risks and benefits associated with Down’s syndrome. As with all discussions of risk, a balanced and fully informed prognosis needs to be presented. As doctors, we are committed to delivering this without prejudice, and as the GMC states, a good doctor should “work in partnership with patients and respect their rights to privacy and dignity. Treat each patient as an individual. Do their best to make sure all patients receive good care and treatment that will support them to live as well as possible, whatever their illness or disability”[5]. The RCOG is running a campaign entitled ‘Every Baby Matters’. This aim is laudable and to be fully supported, but one might be forgiven for being left wondering if, in the light of the RCOG’s positions noted above, this should be renamed ‘Every Baby with 46 Chromosomes Matters’. We, the undersigned, call upon the RCOG to demonstrate that unborn babies with Down’s syndrome matter, and that the College respect their dignity in the following ways:
