CMHC Intake Specialist

Job Description:
This position conducts intake and mental health assessments for new consumers.  Under the direction of the CMHC Intake Supervisor, the position follows up on all referrals, schedules intakes, documents all contacts, maintains referral/intake database, and completes service access reports.  The position also provides referral for those not suitable for programming.  Other duties include coordination with the Transportation Department, CMHC Teams, and consumers to provide case management to ensure consumers begin services.  Marketing and speaking to potential consumers and families in off-site and on-site settings is another component.  In addition, this position fulfills needed functions on the team and may help with group facilitation if necessary.  
Qualified Mental Health Professional (QMHP) with a Master’s Degree in Counseling, Social Work, Psychology, or related field required.
Knowledge and experience with the documentation and billing process for Medicaid community mental health services preferred.
Effective communication skills, both written and oral.
Highly developed organizational skills.
Ability to organize assessment information and inputs from a variety of sources for report writing.
Experience with mental health services community resource networking.
Valid Illinois driver’s license and documentation of current auto insurance, with a good driving record and car available.
Must be willing to work evenings/weekend hours as necessary.
Proficiency in the use of computers and working knowledge of Microsoft Word and Outlook applications, with good typing skills.

Community Residential Supervisor

Minnesota State Operated Community Services (MSOCS) has two full-time openings for dedicated human services professionals to manage the daily operations of residential group homes in the Brainerd area, which support adults with intellectual/developmental disabilities and behavioral challenges. The rewarding Community Residential Supervisor role offers a unique mix of program management, staff supervision, and direct care duties.

Job Description:

This position provides supervision of employees, ongoing direction for treatment service delivery, management of fiscal resources, oversight of professional and other services contracts, and maintenance of the physical environment for assigned program sites. Also functions as Designated Manager, plus Administrator of Records, for each site.

Responsibilities include:

  • Lead staff in the assessment, development, implementation, and evaluation of customized treatment plans that incorporate the wishes, preferences, and goals of each individual;
  • Schedule and supervise employees in compliance with labor agreements and plans so that individuals receive quality services;
  • Manage the budget for each site so individuals receive maximum benefits from monies available;
  • Support clients in maintaining a pleasant home environment according to their preferences, needs, rights, privacy, safety, and dignity;
  • Assist residents to maintain optimal health, along with encouraging them to become involved in community activities, events, and organizations;
  • Develop training for employees and continue professional development as it relates to supervision and developmental disabilities and related conditions.

Minimum Qualifications:

·         Bachelor's degree in a human services-related field AND at least one year of full-time work experience providing direct care services to persons with disabilities or who are at least 65 years old; OR

·         Associate degree in a human services-related field AND at least two years of full-time work experience providing direct care services to persons with disabilities or who are at least 65 years old; OR

·         Diploma in a human services-related field AND at least three years of full-time work experience providing direct care services to persons with disabilities or who are at least 65 years old; OR

·         At least 50 hours of education and training related to human services and disabilities AND at least four years of full-time work experience providing direct care services – under the supervisor of a person who meets the qualifications in one of the categories above – to persons with disabilities or who are at least 65 years old.

AND possess:

  • One year of experience writing or overseeing program plans;
  • Considerable knowledge of the care and treatment of mentally ill, developmentally disabled, emotionally disturbed, chemically dependent, adolescent, TBI, or geriatric clients;
  • Ability to exercise supervision over paraprofessional and direct care staff;
  • Ability to establish and maintain working relationships with professional staff, clients, families and staff of other agencies;
  • Skills in word processing and spreadsheets;
  • Excellent written and verbal communication skills;
  • Valid driver’s license

Preferred Qualifications:

  • Masters Degree in Human Services related field;
  • 3 years multi-site supervisory/management experience;
  • Training experience;
  • 3 years experience working with DD, MI and/or TBI;
  • Experience working with computer based programs;
  • Experience planning service initiatives with county case managers and other DD service providers;
  • Understanding of behavior analysis;
  • Experience working with individuals with higher level behavioral issues.

Service Coordinator/QDDP

Job Description:

This position is responsible for the development, implementation, and facilitation of the Service Plan process for persons served, ensuring integration of service planning through the involvement of the individual, family, guardian, other service providers, agency staff, and others identified by the individual.


Bachelor's Degree in a Human Service field of study required, and must meet all QMRP requirements.
Minimum of one year experience working with individuals with developmental disabilities and/or mental illness.  Effective organizational and communication skills, both written and oral.  Valid Illinois driver's license with good driving record.  Proficiency in the use of computers, software applications, and working knowledge of Microsoft Word and Outlook.
