Step 5: Complete the Electronic Driver Abstract Request Found Below (By Clicking the Link) Step 6: Complete the Medical History Questionnaire Found Below and Visit Dr. Frank to complete Physical form and TB test. Step 7: Submit the Driver Abstract report, Background check report, Medical History Questionnaire, Physical form, and TB test results to membership chairperson. Step 8: Attend a Regular Squad Meeting on the 3rd Wednesday of the month at 7:30pm to be sworn in |
