The Hawaii School Attendance Law requires all students to meet its health requirement:

  • TB Clearance - Students entering public schools in Hawaii for the first time must have current documented tuberculin clearance completed within one year prior to school registration.

All Hawaii State private school transfers must show documentation of x-ray or TB skin test (mantoux) taken sometime while attending that private school in Hawaii . A free x-ray or TB skin test is given on Tuesdays and results read on Fridays between 3:00 pm and 4:00 pm (except on State holidays) at the Wailuku Health Center, located at 121 Mahalani St., Wailuku.   • Physical Examination and Immunizations - Students entering public schools in Hawaii for the first time are required to have these records documented upon school registration or have an appointment by a physi8/20/08

    Students shall have had a medical examination not earlier than 12 months prior to the date of entry to school. The examination report shall indicate that all requirements for immunizations have been met or that notations of those incomplete immunizations have been made.

    The medical examination and immunization records must be certified by a licensed ( United States ) physician. It is recommended that students have a medical evaluation at least every three years.

    • Health Requirements for Athletes - All students participating in intramural and inter-school athletic (MIL) programs are required to have physical examinations annually (12 calendar months). Such examinations shall be within 3 months of the start of the first sport of the year in which they will be participating. Athletic Physical Forms may be obtained from the school office or the Athletic Trainer's office.
