WHEN IS THE CLINIC?

TUESDAY, DECEMBER 18, 2018 from 3:00pm -5:00pm the JA Parent Group is partnering with Beacon Health System to offer an on-campus flu shot clinic. The cost is $25 per person payable in cash, credit or money order. NO PERSONAL CHECKS ACCEPTED. Payment will be collected by Beacon staff before receiving the shot. Beacon provides the nursing team for the South Bend School District.


WHO CAN GET A FLU SHOT?

All students, parents and any family member along with faculty and staff are welcome to attend the clinic who are older than 4 years old. JA students will be excused from class if they are signed up and have a consent form if needed (see next section).


IF MY STUDENT IS UNDER 18 DOES A PARENT NEED TO CONSENT?

YES. The consent and information forms can be opened by CLICKING HERE. This needs to be completed and signed by the parent/guardian for anyone under the age of 18 on the date of the clinic. The student must bring the signed consent form with them to get the flu shot.

WHERE WILL THE CLINIC BE HELD?

It will be in the John Adams Auditorium. If a parent or family member is attending please enter from the main door #1 under the Tower on Twyckenham. Security will have you sign in as normal to get a visitor pass.


SIGN UP:

In order to have sufficient flu shots available please register by filling out the online form by CLICKING HERE. A separate registration must be completed online for each person getting the flu shot.


EXIT PASSES:

For those students who sign up they may get an exit pass from Student Managment in order to attend. If your student is pre-registered the Student Management office will have access to this online sign up list so no note from home is needed.








GENERAL HEALTH GUIDELINES FOR PARENTS TO KNOW:
1) WHEN SHOULD I KEEP MY STUDENT HOME( i.e. flu guidelines)?
  • Please do not send your children to school if they are sick. 
  • A fever over 100 F is a reason to keep your child at home. 
  • Students should stay at home for 24 hours after the fever has stopped.
  • Please read the attached  letter from the Health Services Department (in Spanish or in English) which has more information about procedures that the school will follow and that you should follow if your child becomes ill.
2) WHAT VACCINES ARE REQUIRED?:
All students must have their vaccines up to date as well and provide an updated immunization record to the school nurse. If vaccines are not update within 20 days of starting school, you'll receive a letter notifying you of the possibility of your student(s) be excluded from school until they have been updated.
