Birth and Death Certificates

Birth & Death Certificate Registration in Jackson County begins in 1882, but was not required  registered until 1907.

Birth Certificates: 
 
Only persons born in Jackson County are able to obtain a birth certificate through the Jackson County Health Department.
 
Apply In Person
 
Orders are processed while you wait upon completion of a birth certificate application and approved proof of indentification.
 
Apply Through Mail
  
    To apply by mail, print the above application or send the following information:
    Your full name at birth.
    Your date of birth.
    Your mother's full maiden name.  (If adopted, give adoptive mother's full maiden name).
    Your father's full name.  (If adopted, give adoptive father's name).
    If there has been a legal adoption, give us all the information that you know prior to the adoption.
    The reason you need the birth certificate.
    Sign your request with your legal signature, address and a daytime phone number.
    A copy of your drivers license or identification with your signature on it.
    A stamped, self-addressed envelope (5x7 or larger).
    Enclose a money order payable to:  The Jackson County Health Department.
    Mail all to: Jackson County Health Department, 801 West Second Street, Seymour, Indiana 47274
    
 
    The Jackson County Health Department offers a variety of birth certificate options. 
 
    Standard size birth certificate in vinyl sleeve                            $10.00
    Wallet size birth certificate in vinyl sleeve is                            $10.00
    Combo (1-wallet size & 1- standard size in vinyl sleeve)           $15.00 
    All birth certificates are certified with the raised county seal.

Death Certificates:

 

To obtain a death certificate in our Health Department, the individual must have died in Jackson County.

 

 If you come to the office you will need to fill out an application with the decedent's information and your name, address and telephone number.

 

If you are not able to come into the office you may apply for it by mail. You will need to send the following information:

Your Name

Address

Daytime Telephone Number

Date

 

Name of Deceased

Date of Death

Place of Death

Reason Needed

 

Enclose a money order payable to Jackson County Health Department. Certified Death Certificates are $15.00 each
