Total Knee Arthroplasty in Kashmir - A Prospective Study

Abstract

A prospective study was done between the periods of may 2013-June 2016.20 patients who consented were enrolled in this study and were assessed clinically, f unction ally and radio logically using Knee Society Score. The majority of the patients were from the age group of 61-65 years which accounts for 30% of patients in our study .The youngest patient was 43 years of age and the oldest patient was 75 years. The mean age was 58.10 years. There was a female predominance, accounting for 55% of the patients. Right side was predominantly involved accounting for 55% of the patients. Osteo-arthritis accounted for majority of the patients (90%) and in 10% patients the affection was Rheumatoid arthritis. The average preoperative knee clinical score was 30.10 which improved to an average post-operative score of 93.3.This was first study of its kind in Vale of Kashmir and main limitation of the study was small sample size and in future we need to include large sample size over extended period.

Cemented Total Hip Replacement, a Jammu Kashmir observation

Abstract

One hundred one consecutive harris design-2 total hip replacements were performed,between april 2005 and march 2016 by two surgeons(JAB and NAM).The mean age of fifty one men and fifty women at the time of operation was sixty-three years and six months(range fifty nine to ninety two years).Right side was involved in sixty one hips and left side was affected in forty cases.The acetabular component was all polyethylene with an eccentric articulation that maximized the thickness of polyethylene in the superior and posterior quadrants.The femoral component was available in three sizes(small,medium and large) but did not have proportional dimensions since the length of the stem remained constant for all stem sizes.The femoral component had a collar in twenty eight cases,a matte finish,and a rounded rectangular cross section;in other cases the stem was collarless. In our series,in which the duration of follow-up was 5-11 yrs(mean 10 years),the clinical and radiographic results of Harris Design-2 total hip replacements performed with so-called first-generation cementing techniques are comparable to those reported for implants inserted with first-generation technique.

Favourable outcome of Cementless Total Hip Arthroplasty

Abstract 

Background: Total hip replacement has become a standard treatment for patients exhibiting hip joint deterioration by a number of causes. Indian surgeons annually perform more than 150,000 total hip Arthroplasties (THAs), 90% of which are primary procedures. The optimal method of fixation for primary THA, particularly fixation with or without the use of cement is still controversial. Improved surgical technique and instrumentation have expanded the clinical indications for Cementless THAWe started study to evaluate outcome of Cementless Total Hip Arthroplasty. All the patients were evaluated clinically (according to Modified Harris Hip Score) and radiologically at 6 months post operatively and were followed up to 3 years. The patients operated in our study had an average Modified Harris Hip Score 97.25. Conclusion: Results of Cementless total hip arthroplasty are excellent and good. Cementless THA Showed encouraging results in short term follow up but their long-term results are yet unknown. These data will promote use of Cementless THA in younger population. Although cost of Cementless THR is high, but in younger population it is better option because of bone ingrowth & ongrowth surfaces giving more stability. If required at later date Cemented THA can be done. Cemented prosthesis although cheaper, complications like loosening due to osteolysis is associated with poor outcome in younger population. Further removal of cement is difficult while revising cemented THA.

Abstract 

Presented herein are the analytical studies of lubrication mechanism occurring in knee joint replacement under restricted motion. The idealized model is to produce the result consistent with those in the normal situation. We observe that the axial pressure increases with the increase the value of x and axial pressure decrease with the increase of value of h. We observe that the load bearing capacity increase with the increase in the value of viscosity. We have also observed that the load bearing capacity decreases as with the increase of the value of film thickness.
