This SSS Maternity Notification form is the first document that you will
submit to the SSS, so you can claim later on your Maternity Benefit.
If you're Employed, submit an accomplished form to your employer.
If you're Voluntary, Self-Employed or OFW, submit it to an SSS branch at least 60 days from the date of conception but NOT LATER than the date of delivery.
Include a copy of your ultrasound report or certification of pregnancy from your doctor.
An SSS officer will stamp your MAT1 form and give it back to you. You will keep this stamped form, and submit it together with other maternity benefit claim documents to the SSS after you have given birth.
