1. Employer Registration Form - R1
You will get an Employer No.
2. Employer Report Form - R1A
You will register your household helpers as under your Employer No.
3. Employer Contributions Payment Form - R5
Use this when you pay your contributions for your helpers
4. Contribution Collection List - R3
Use this to report your payment of contributions. Submit together with your R5 payment already stamped by bank or collector
5. Miscellaneous Payment Form
Use this to pay your Employer Registration Form plate and sticker
