These are the forms you need to accomplish and file with SSS so you can pay SSS contributions for your employees.
These can be used by companies or household employers.
Companies need to submit additional documents.
1. Employer Registration Form - R1
You will get an Employer No.
2. Employer Report Form - R1A
You will register your employees
3. Employer Contributions Payment Form - R5
Use this when you pay your contributions for your employees
4. Contribution Collection List - R3
Use this to report your payment of contributions. Submit together with your R5 payment already stamped by bank or collector
SSS announced that starting July 1, 2014, it will no longer accept hardcopy or paper-based contribution collection lists (R-3) and loan collection lists (ML-2) from employers.
5. Miscellaneous Payment Form
Use this to pay your Employer Registration Form plate and sticker
