Strive for excellence What we do Older adults and others currently rely on Ebenezer and Fairview for senior housing; transitional and long-term health care; adult and intergenerational programs; home and community-based services; and the compassion and respectful care for which we are known. Senior Housing Ebenezer provides management support and services for many independent, assisted living and memory care senior communities throughout Minnesota. Each site offers a range of services such as meals, transportation and recreational activities, assistance with housekeeping and laundry, social services and short-term rehabilitation following surgery. Health Care Facilities When health care needs can no longer be met at home, Ebenezer provides a safe and highly supportive environment where residents can be as independent as possible. We work closely with residents and their families to develop a care plan tailored to best meet their needs. We also provide transitional care, or short-term skilled nursing care, for those recovering from an injury or illness. These specialized units are designed to help patients get the rehabilitation and extra support they need to meet their care goals and return to their homes. Adult Day Programs Our adult day programs provide services for older adults who live at home or with family, but need some assistance during the day while caregivers are at work or in need of respite. Our programs include a structured daily schedule; individualized care plans; nutritious meals; an atmosphere of social and spiritual enrichment; and a circle of new friends. Intergenerational Day Program The intergenerational day program in Burnsville combines both adult day and child day care, meeting the needs of the youngest and oldest members of our community and their families in a unique way. Young and old gather daily to share activities, have fun and form meaningful relationships. Home & Community-Based Services From senior companion programs to home health services, Ebenezer and Fairview offer a range of coordinated services to meet the needs of individuals who choose to continue living at home.
