Line 1 Enter purchaser(s) name and complete property address, including county. □
Line 2 Enter name(s) and style (type of tenancy) in which title will be taken. □

Enter bid amount and earnest money amount that purchaser has deposited.
• Fill in Earnest Money amount of $500 for a bid of $50,000 or less or $1,000 for a bid of $50,001 or 
more in the “Purchaser has paid $ __________” area.   
• Fill in “The earnest money deposit shall be held by HUD’s Designated Closing Agent.”
