Committee Forms/Guidelines
Below are the forms and guidelines you need as a Hopewell PTA Committee Chair.  The documents are listed below and are downloadable.

Financial Forms

Below is a "PTA Financial Forms-1" file containing the "Funds Collected", "Request for Funds", and "Cash Box Request" form.  Please use these forms when turning in money collected for the Hopewell PTA, when you are requesting to be reimbursed for a PTA expense, or when you need a cash box for one of the PTA events. 
Collected Funds: Please count the money collected with another PTA member, complete the "Funds Collected" form, and notify the treasurer you have money to be deposited.  Funds are NOT to leave the school and should be locked up.  The Hopewell office secretaries can assist you with locking up the collected money. If you have any questions, please contact the treasurer.
30 Days to Turn in Receipts: To receive reimbursement from a PTA event, receipts need to be turned into the treasurer with the completed "Request for Funds" form.
Cash Box Requests should be made at least 5 business days before the event.
