We are pleased to file ALL dental insurance claims as a courtesy to our patients. Please bring your insurance information with you to the initial visit so that we can assist you in receiving your maximum benefit. 

We do recommend you familiarize yourself with your specific insurance benefits prior to visiting the office. We will work hand in hand with you to maximize your insurance reimbursement for covered procedures. However, the ultimate responsibility for payment lies with you. If you have any problems or questions, please ask our staff. Remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.                  


Time is very important. Your appointment slot is reserved for you. We do not “double-book” in order to give you the best treatment possible. Because of this, we request you give our office 24 hours notice if you are unable to come to your appointment. Failure to do so more than one time may result in a terminated patient status. 

ALL patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visits.
