Volunteers Needed to Help at Walk Thru

Volunteers are needed to help Walk Thru run as efficiently as possible for our families. Please consider volunteering for a shift. No experience necessary - you will be trained prior to your start time. You can sign up by clicking here.

Forms to Bring to Walk Thru

Download and complete the following forms at home, then bring them with you to turn in at Walk Thru. Most forms need signatures on them.

  • Dance Permission Slip

  • Computer Use Agreement

    • For your protection, please read this agreement. The signature page has been mailed home to you and must be signed by your child and you in order to receive your child's schedule. If you did not received it, please print it here.

    • Please only bring the signature page.

  • Film/Video/Photographic Production - this form is giving Foothill Middle School permission to post your child's picture or video on the following sites:

    • Our webpage

    • A Drama production CD

    • A newspaper article

    • Our Journalism "Stitch" production

  • Walk-Thru Registration Draft Receipt - Click here for a sample of the Walk-Thru Registration Receipt. This form will be completed on campus the day of Walk Thru. It includes information about:

    • Class Donations and Fees - Many of the classes request a donation for materials and supplies. Class donations must be in the form of a personal check or cash. Unfortunately, credit cards are not accepted.

    • PFA Educational Fund - A suggested donation of $295 per student. Credit cards, cash or check are accepted. You can make an online donation via Paypal prior to Walk Thru if preferred. To learn more about the PFA Educational fund, click here.

  • Directory - To have your personal information included in the directory for the 2015-16 school year, please click here. Complete and submit the form online.

  • Volunteer Sign-up Form - If you would like to volunteer your time so Foothill will remain an outstanding school, please click here. Complete and submit the form online.

  • School Supplies List

  • Fingerprinting and TB Testing information for volunteers

  • Authorization to Administer Medication During School Hours - This form can be found on the district website. Click here to complete and print the form. Please return the completed form, signed by your doctor, to the office along with the medication clearly labeled with your child's name.
