YOU MUST HAVE THESE 5 FORMS COMPLETED AND ON FILE BEFORE YOU CAN BEGIN TO PRACTICE WITH THE TEAM:

1. Assumption of Risk- If you have not played a sport for EWG HS already, you must have this form filled out and NOTARIZED.
2. Physical- Current for one calendar year. Make sure your physical has not expired.
3. Concussion Awareness- Please read this brochure, then fill out and hand in the form section. Understanding concussion injury is critical.
4. School Insurance- You MUST purchase school insurance, even if you are covered with your own health care plan.
5. EWG Athletic Handbook- You and your parents/guardians must read this handbook, then print out, sign, and hand in the last page.
