These can be found on East's website at: Go to Athletics and Activities and download all forms.  ($60 pay to play fee must be turned in to the Treasurer at East.)  Also, a Doctor's approval is required.  Here is a link for optional physicals: 

  General information regarding the Boys Tennis season:

The boys' tennis season is very short--mid-August to mid-October, with most players finished by the end of September.  A great deal gets packed into these eight weeks.  We typically have between seventy and eighty boys come out for tennis each year, ranging in ability from beginners to accomplished tournament players.  No one is cut.  Right out of the gate, our first task will be sorting.  Our strongest eleven players make up our Varsity One squad, comprised of three singles players and four doubles teams.  Our next eleven make up Varsity Two.  All the rest of our players play Junior Varsity, which is almost entirely doubles.  Both varsity squads will play full schedules of twelve competitions (dual matches and tournaments).  Junior Varsity players average 5-6 competitions.  We practice and play home matches at the City Park courts, in the northwest corner of the park off 23rd.  Please visit the East High website to download the athletic forms that must be turned in before we can allow players to participate. 

 There is a fee of $60 in accordance to the "pay to play" guideline.  Payment is to the East Treasurer on the 2nd floor, next to mail office in the school. The team will be going as a group to turn in all paperwork and payment on Monday, August 11th at 12:30 if you have not do so beforehand.  Watch for emails on date and time.  Questions about payment?
