Our Services

Divine Connect Care, Inc. provides standard specialized services and supports 24 hours a day, 7 days a week. However time period for supports is based upon individual need. 

In-home Support

Divine Connect Care, Inc. assist participants with a combination of hands-on care, habilitative support, and assistance with activities of daily living.  In-Home support services include the following areas:

  • Training and support in activities of daily living and independent living skills.
  • Assistance in performing personal care tasks.
  • Assistance with light household tasks specific to the needs of the person.
  • Assistance with homemaking tasks such as food preparation and laundry that are specific to the needs of the person.
  • Training and support on understanding and utilizing community resources.
  • Training on, and assistance in the monitoring of health, nutrition, and physical condition.
  • Training and support in adapting to a community and home environment, including management of financial and personal affairs, and awareness of health and safety precautions.
  • Coordinating transportation to community events.

Supported Living

Divine Connect Care, Inc. assist participants in the acquisition, retention, and improvement of skills related to activities of daily living, such as personal grooming, household chores, eating and food preparation, and other social and adaptive skills necessary to enable the person reside in the community.


Respite

Respite care services are provided by Divine Connect Care, Inc. to participants unable to care for themselves. Respite care is furnished on a short-term basis because of the absence or need for relief of those persons normally providing the care. Respite care will ensure that participants have access to community activities as delineated in the participant’s Plan of Care including transportation to and from the activities the person would receive if they were not in respite.

Host Home Services

Divine Connect Care, Inc. support and assist participants with daily living skills and other individual needs in a manner similar to the residential program, however, this service is provided in the caregiver’s personal home.Host Home services include the following areas:

  • Room and board
  • Light homemaker tasks, such as assistance with meals preparation.
  • Light chore tasks, such as assistance with laundry, shopping, and general housekeeping.
  • General supervision of the person as described in the ISP and Plan of Care.
  • Maintenance of medical records.
  • Maintenance of financial records.
  • Maintenance of ISP and Plan of Care.
  • Assistance with attending health care appointments, including coordinating but not providing, transportation to and from appointments.
  • Assistance with planning and attending community events.
  • Providing habilitative support in activities of daily living and/or therapeutic goals and objectives as described in the ISP and Plan of Care.
