The following guide will help you and your unit assure that your application is complete correctly prior to submission to the council service center. Do not submit this checklist with the Eagle Scout Rank Application.
We suggest that you print this page and use it as a check list. Any incorrect data or omission will likely delay your advancement.
It is important to note that once a Scout has been advanced in rank, advancement errors subsequently discovered must not be held against him/her for any future advancement, even if the requirements were not properly completed. As an example, if after a Scout was advanced to Life rank it was discovered he/she had less than the required six months between Star and Life, that would not be cause to deny his advancement to Eagle.
You should not submit this checklist or Part 6 of the Eagle Scout Rank Application to council. Part 6 will be inserted in the Eagle Board of Review binder that goes to the district.
□ 1. Used the current version of the official Eagle Scout RankApplication, No. 512-728. Council will reject earlier versions and will force you to resubmit the correct application.
□ 2. Applicant’s full legal name, address, phone number and email are correct.
□ 3. Unit type, local number, and location are entered. No abbreviations.
□ 4. Date joined Boy Scout troop is entered. All dates should match those submitted previously for rank advancement.
□ 5. Date became a Varsity Scout is entered (if applicable).
□ 6. Date became a Venturer is entered (if applicable). See also “Boy Scout Advancement in Venturing and Sea Scouts,” 4.3.1.4.
□ 7. Date of First Class Scout board of review entered.
□ 8. Date of Star Scout board of review entered.
□ 9. Cub Scout, Webelos Scout, Arrow of Light, and fifth grade questions answered.
□ 10. Date of birth entered.
□ 11. Life Scout board of review date entered.
□ 13. Must list all six (five if not employed) references. If not affiliated with an organized religion, then the parent or guardian provides religious reference. The Scout may list anyone he/she chooses for the remaining two references, including parents or guardians not previously listed, other relatives, Scout leaders including those from his unit, or other Scouts and friends. There is no requirement that any of the references be 21 years of age or older.
□ 14. The Board Chair is delegated the responsibility of securing recommendations from the references appearing under requirement 2 on the Eagle Scout Rank Application. This may be done by letter, form, or phone call. For reasons of privacy and confidentiality, electronic submissions are discouraged. It is acceptable to send or deliver to the references an addressed envelope with instructions, and form to complete. The Scout may assist with this, but that is the limit of his participation. He/she is not to be responsible for follow-through or any other aspect of the process.
It is the Board Chairs responsibility to collect the responses. If after a reasonably diligent effort, including telephone calls, no response can be obtained from any references, the board of review must go on without them. It must not be postponed or denied for this reason, and the Scout shall not be asked to submit additional references or to provide replacements.
Completed reference responses of any kind are the property of the council and are confidential, and only review-board members and those officials with a specific need may see them. The responses are not to be viewed by or returned to the Scout. Doing so could discourage the submission of negative information. For the same reason, those providing references do not have the option of giving the reference directly to the Scout and shall not be given the option of waiving confidentiality. Once a review has been held, or an appeal process conducted, responses shall be returned to the district advancement representative, where they will be destroyed after the Eagle Scout credentials are released or the appeal is concluded.
□ 15. Twenty one (21) merit badges were completed at least one day before the applicant’s 18th birthday and are listed with date and unit number on each. Badges 13 – 21 should reflect badges that have previously been recorded for earlier rank advancement. Badges should be entered in the order they were earn for rank advancement (i.e. don't start with the most recent or favorite badge)
□ 16. On merit badge line 6 and 9, merit badges that do not apply have a line drawn through them. If any of the marked off merit badges are to be counted as part of the 21 total merit badges, the badges must be put in one of the other boxes (13 through 21).
□ 17. Four (4) Eagle required merit badges have a date prior to the Star Scout board of review date. Credit may be given if the Scout earned both Emergency Preparedness and Lifesaving or any combination of Swimming, Hiking, and Cycling.
□ 18. Two (2) additional merit badges must have a date prior to the Star Scout board of review date.
□ 19. Three (3) Eagle required merit badges have a date prior to the Life Scout board of review. These Eagle required merit badges are in addition to the ones earned for Star Scout rank. Credit may be given if the Scout earned both Emergency Preparedness and Lifesaving or any combination of Swimming, Hiking, and Cycling, as long as they are not used for credit for Star Scout rank.
□ 20. Two (2) additional merit badges have a completion date prior to the Life Scout board of review. These merit badges are in addition to those earned for Star Scout rank.
□ 21. Date of Life Scout board of review entered on second page of the application and matches the Life Scout board of review date on the front page.
□ 22. Has served for a combined period of 6 months in one or more qualified position(s) of responsibility listed in Eagle Scout rank at least one day before the candidate’s 18th birthday. The position(s) must relate to the unit where the Scout was registered and active at the time service was rendered. For example, “SPL” would not be used by a crew member unless the Scout was, or is, also registered in a troop. For a Scout who has transferred from a troop or team to a Venturing crew or Sea Scout ship, any qualifying position(s) held after the Life rank board of review in the troop or team shall count, and the six-month element of the requirement may be met through a combination of non-concurrent positions served in the Troop, Team, Crew, or Ship. See also “Boy Scout Advancement in Venturing and Sea Scouts,” 4.3.1.4. or an accumulated period of at least 6 months between the Life Scout board of review and the Eagle Scout Rank Application date. Do not overlap position dates. The start of position may begin before the Life Board date however on the application; no position should be listed that occurs before the Life BoR date.
□ 23. The date of final signature for the Eagle Scout Leadership Service Project is the same date as the date on page 20 of the Project Workbook. Date must be between Life Scout board of review and Eagle Scout Rank Application dates and at least one day before applicant’s 18th birthday.
□ 24. The Life Ambition and Purpose Statement form lists the life purpose/list of leadership positions/ honors and awards should be reviewed, dated and signed by the Unit Leader. Note: This form is not submitted to the service center.
□ 25. (It is the responsibility of the Eagle Candidate to make sure that the original completed Eagle Scout Leadership Service Project Workbook and Statement of ambitions/life purpose/list of leadership positions/ honors and awards are brought to the Eagle Board of Review.)
□ 26. Scoutmaster conference date entered and signed at least one day before applicant’s 18th birthday.
□ 27. Applicant’s signature, applicant’s telephone number, and date signed entered.
□ 28. Unit leader’s signature, unit leader’s telephone number, and date signed entered.
□ 29. Unit committee chair’s signature, telephone number, and date signed.
□ 30. All information is legible.
□ 31. A candidate must be registered through the time he/she is completing requirements but need not be registered thereafter or when his board of review is conducted.
