Located below are links to all of the forms and information that you need to submit student names for consideration into District Orchestra. Please do not use forms that you have obtained from any other sources. Using the incorrect forms will render your students ineligible for acceptance into District Orchestra. Jennifer Helms Shaffer, Orchestra Coordinator, can guarantee that the forms linked below are correct.

Please make sure that all paperwork is completely filled out. Leaving spaces blank will render students ineligible for participation in District Orchestra. Even the tetanus shot date must be filled in.

Remember that a school DOES NOT have to have an orchestra program to have students who are eligible to participate in District Orchestra.

Any student attending Regional Orchestra the proceeding year should be considered as an automatic return only if the director has indicated them as such on the preference form and the proper forms have been submitted. The selection process will continue after all Region students have been placed. Participation in prior District Orchestra festivals does not count for any automatic returns.
