Make appointments for preventative care (physicals, routine check-ups) as far in advance as possible to allow you the best selection of dates and times.
Be prepared to tell the appointment receptionist which provider you wish to see, and the reason for the visit. This allows the receptionist to make certain you get the right time and attention you deserve. Any information you give to any member of our staff is held in strictest confidence.
Providers typically have special availability for patients with acute (sudden) illness. Such "sick" visits allow patients to be seen sooner than patients who need routine care. Tell the receptionist if this is your situation. If your provider is away from the office or has already completely filled their schedule, you are welcome to see another provider who has an available appointment. You may be sure your primary provider receives information about your visit from their associates. Nurses are available to assist you and the receptionist in determining the right provider and time if you have questions.
When you come to Cottonwood Medical Center for each appointment, bring a picture ID (state issued ID card, drivers license, or Passport) and your insurance ID card(s). We require this to prevent identity theft and insurance fraud. For more information, please see
