FORMS REQUIRED FOR EVERY SPORT, EVERY SEASON:Permission Form
Athletic Fee Schedule - SPORTS FEES are $150 first sport, $150 for second sport and the third sport is FREE. (Fees are adjusted on this form if you qualify for free & reduced lunch.) Annual ASB FEE $30 also required.
FORMS REQUIRED ONCE PER SCHOOL YEAR:Athletic Code (This code has been revised and needs to be read and signed by both student and parent)
Athletic Clearance Form/Participation Fee Refund Form (This is a 2-sided documents, by signing the Athletic Clearance form you acknowledge you have read, understand and comply.)
PHYSICALS:Physical Form (Every incoming 9th grader must have a physical. Repeat physicals are required every two years. Physicals must be on this state mandated form and turned in, signed by the students’ physician, at the time of registration.)
INSURANCE:Insurance - ALL ATHLETES MUST HAVE INSURANCE COVERAGE IN ORDER TO PARTICIPATE - School Insurance is available. We can NO longer instantly validate insurance coverage when purchasing school insurance. We must fax form and copy of check to company and they will send us validation the following day.
FINES:Fines (All fines must be paid in full before you will be eligible to compete. Click here to go online to see if your student has fines.)
“Blue Slip” for participation will be issued by the school when all of the above necessary forms have been returned, fees and fines paid, insurance is in place, grades and eligibility have been checked.Pay Fees Now Online (click her to pay your fees)
