General Execution Checklist

We are trying to keep the administration of this program as simple as possible.  We are, however constrained by various requirements which we have no control over.  This checklist will take you through the steps to cover all the bases.  It really is not as bad as it looks at first glance.  

All volunteers must complete the following before participating
  • Verify your eligibility (see below), availability (see Timeline)  and check with your supervisor/mentor/employer
  • Complete the sign-up form [See Sign Up Form]
  • Travel Orders  [See Travel Orders for instructions]
  • Health Assessment Form [See Medical Screening & Emergency Contact]
  • Volunteer Agreement Packet (non-federal civilians only - may include certain graduate students) (see below)
  • Parking pass and placard (if needed)
  • Prepare your personal gear [see packing list]
  • Muster at the USU flag pole at 1815 the evening before your role-play duties


Eligibility - Who can volunteer?

We would like for ALL of you that are interested to participate.  However, we do have legal constraints which we have to honor, most of which are based on your employment status within the University.  

Military service members on active duty, Administratively Determined (AD) faculty, General Schedule (GS) and Federal Wage System (FWS) federal employees are all eligible to volunteer with approval from their supervisor. 


  Military Personnel

No restrictions. Immediate supervisor must approve.
  Administratively Determined (AD) Faculty and Employees

No restrictions. Immediate supervisor must approve.
  General Schedule (GS) and Federal Wage System (FWS) Federal Employees

General Schedule (GS) and Federal Wage System (FWS) federal employees are eligible to volunteer with approval from their supervisor. Paid overtime is not authorized for this training exercise. GS and FWS employees who volunteer to participate in the exercise are authorized compensatory time off, if the employee works over their regular scheduled eight (8) hour tour of duty. Depending on the participation date, GS and FWS employees who volunteer to participate in this exercise will be required to revert to a regular eight (8) work schedule for the pay period beginning on Oct 5, 2014 or Oct 19, 2014.

  Graduate Students (Military) Eligible to support, but must clear with your faculty mentor and program. 

  Graduate Students (Civilian)  Eligible to support, but must clear with your faculty mentor and program. Requirements for non-federal civilians may apply.


Non-federal Civilians Non-federal civilians are eligible to volunteer, but must complete a USU volunteer Agreement Packet.  [See below]



Volunteer Agreement Packet

Volunteers in the following categories must also submit a volunteer agreement package to bushmaster-ggg@usuhs.edu or to the MEM team in C1039.   301-295-9636

Those requiring the Volunteer Agreement Packet are:

(1) Federal employees acting in their personal capacity (outside scope of normal duties)

(2) Retired Military Personnel (with no current federal connection);

(3) Private civilian employee 

(4) Contractor acting in their personal capacity 

Any others do NOT need to complete this package.

The packet includes the following:
  • DD2793 Volunteer Agreement  download
  • CV/Resume
  • Identification Verification (copy)
    • Option 1: US Passport
    • Option 2: Birth Certificate,Valid Driver's License Retired Military ID AND SSN Card
  • FTIG Release and Hold Harmless Agreement  download adult version  download child version
