Health and Medical Record

Sea Base participants must be in good health. All participants must provide
a current and complete BSA Annual Health and Medical Record signed
by a physician. BSA Annual Health and Medical Records are good for 12 months from the end of the month in which they were completed. The form can be found on the Sea Base website www.bsaseabase.org/resources/forms

No other form will be accepted. Out of date forms will not accepted.

First Aid and CPR Certification

It is required that one adult leader in each crew completes Wilderness First Aid & CPR or hold a higher professional certification. Multiple crews require multiple trained leaders. It is strongly recommended that every adult leader completes Wilderness First Aid and CPR and that every youth completes Wilderness First Aid or Basic First Aid and CPR.
