Welcome! 
If your child will soon be entering school,
start planning ahead for the enrollment process. 
You will need the following documents: 
CERTIFIED STATE birth certificate
 Shot record (complete)
Physical / Healthcheck (must be within the year)
 DENTAL exam

If your child has any special health needs, you will need to contact your school nurse to obtain information needed for school enrollment.  There are specific forms that your health care provider will need to complete if your child has special health issues. 
You can contact the school nurse for the school your child will be attending or contact the Director of School Health Services if you have additional questions.
 
    Dental Exams and HealthCheck Physicals are required for:
First time entrants (PK or K)
2nd Graders 
7th Graders
12th Graders 

"ADOLESCENT IMMUNIZATION CAMPAIGN"  for 6th and 11th graders begins in January PRIOR to their 7th and 12th grade years
BRING IN AN UPDATED SHOT RECORD!!!!!!

TDAP and Meningococcal vaccines are mandatory BEFORE the first day of 7th and 12th grade
  (12 graders need to obtain a booster to the meningococcal vaccine if the first dose was prior to 16 years old.)

School Wellness Investigations (ASSESSMENT TOOL)




 

Fuel Up to Play 60 is a program founded by the National Dairy Council and NFL, in collaboration with USDA, that empowers students to take charge in making small, everyday changes at school. FUTP 60 encourages youth to eat healthy and move more; studies suggest that well-nourished, physically active children can be better students.

Boone County Schools participates in this program which helps them meet their wellness objectives and allows students the opportunity to earn rewards and prizes. To promote the importance of breakfast and physical activity, all schools were awarded FUTP60 grants in 2013-14 and several were awarded in 2014-15. We are continuing the various breakfast programs that were initiated at that time to increase our breakfast participation rate.

 

 

Did you know…..

 

Pre-prepared food products are being made with healthier ingredients, less fat, sodium and sugar. Pizzas contain whole grain crusts. Chicken nuggets are baked and have whole grain breading. French Fries are prepared without trans fat and baked. Sweet potatoes are being served.
