From 1916 Millicent was attached as an orderly to a unit of the Scottish Women's Hospitals for Foreign Service and worked from March 1917 at the ancient Abbaye de Royaumont, Asnières-sur-Oise, France. She was sent to the advance hospital at Villers-Cotterets, Aisne, which was taken over by the French military and became Hôpital Auxiliaire d'Armées No.30. There she first experimented with drama. Written partly in English and partly in French, and solely as entertainment for the wounded, her pantomimes, melodramas and variety shows were performed by staff and some of the casualties, using makeshift props and costumes. In the face of the German advance in May 1918, the hospital was evacuated to Royaumont: Miss Armstrong was awarded the Croix de Guerre for her bravery in rescuing wounded soldiers while under fire.

In June 1917 she joined as a voluntary worker the 'American' Unit of the Scottish Women's Hospitals for Foreign Service stationed at Ostrovo, Macedonia, and commanded by Dr Agnes Bennett and Dr Mary De Garis, a stimulating but debilitating experience for 'Franky Doodle', orderly. She returned unwell to London in February 1918, apparently not enticed to stay by the possibility of a paid post as cook on a twelve month contract.

With the Scottish Women's Hospitals for Foreign Service in the Serbian/Bulgarian war - Photos taken by Dr Agnes Elizabeth Lloyd Bennett,

The 7th Medical Unit of the Scottish Women's Hospitals for Foreign Service attached to the Serbian army known as the American Unit, and commanded by Dr Agnes Bennett. This consisted of an ambulance unit and a field hospital. The images record wounded being brought in, views of the hospital camps and dressing stations in Serbian and in Macedonia at Dobraveni, Skocivir, and Ostrovo which seems to have been the main center for the unit's medical operations. There are photographs of the nursing staff, of Serbian military personel, and  of French engineers constructing a bridge over the Brod River, of Lake Ostrovo and Ostrovo Township, of Vodena (Edhessa) east of Ostrovo, and the town of Monastir (Bitola) in Macedonia which was the focus of one of the battles in the Serbian-Bulgarian war in March 1917. Several photographs show nurses celebrating May Day at the camp at Ostrovo. Others show Soldiers graves, a Serbian military funeral, two people watching the bombardment of Monastir from the summit of Mount Kaymakchalan, an amunition dump at Pelatino, a bombed church at Brod in Serbian Macedonia, Bulgarian guns captured by the Serbs, and samples of the local populations both Christian and Islamic.. In 1916 Agnes Bennett was appointed commanding officer of the 7th Medical Unit of the Scottish Women's Hospitals for Foreign Service. From August 1916 her field hospital was attached to the Serbian army. An attack of Malaria led to her resignation in October 1917.(info from The Dictionary of New Zealand Biography, vol three, 1901-1920..
