1. How do I contact you for an appointment?

2. Where are you located?
1282 Timberlane Road, Suite K, Tallahassee, FL 32312 in the Timberlane Commons office complex.
 
3. Do you take insurance?
YES.  I am currently a provider for: 

State of Florida EAP services through E4Health
TRICARE
E4 HEALTH EAP, Incorporated
BLUE CROSS/BLUE SHIELD 
Value Options
Cenpatico
Other insurance: contact your insurance provider and ask if you are covered for mental health care, have met your deductible, etc.

If you have insurance questions I STRONGLY encourage you to go to the website of my medical biller:  www.tghbs.com.  Click on the patient link on the left, then on the drop down menu find my name, click on it and complete the information. Within 48 hours my biller will let us both know what your insurance company will or will not cover.

4. What is your fee?
- All appointments are one hour and $100.00. 
- Paid by exact amount cash, check, debit/credit card. 
- If you have insurance I will bill your insurance provider for your reimbursement. 
- If you do not have insurance you will be asked to pay the fee in full. 
- This fee is due in-full at the end of each session, unless prior arrangements have been made, and is subject to change at any time.
 
5. What are your office hours?
My office hours are by appointment only. Call or email me to set up an appointment that fits your schedule.
 
6. Do you see children or adults?
I see children, adults, couples, groups, and families. 
 
7. What happens if I make an appointment that I cannot keep?
Please call at least 24 hours in advance of any cancellation. There is a $50 fee for missed appointments that are not cancelled 24 hours in advance.
 
8. Tallahassee is a small town and I don't want anyone to know I'm getting counseling. So, what happens if we run into one another in the community?
I will never publicly acknowledge that I have a therapeutic relationship with you. I will not introduce you to my family or friends, and you are under no obligation to introduce me.

9. Is what I say in a counseling session really confidential?
Yes, what you say in a counseling session is confidential with these exceptions that are outlined by the State of Florida Department of Health and the Code of Ethics followed by all counselors:
  • If a counselor feels that a client may do harm to themselves
  • If a counselor feels that a client may do harm to someone else
  • If a counselor feels that a client is harming a child or an elderly person, or someone who is in a vulnerable population
  • If a counselors records are subpoenaed
