HARTLAND COMMUNITY NURSE

The Community Nurse available for home visits to assess, support and referral to appropriate resources.

The Community Nurse is not a care giving position – no bandaids or splints.  The Community Nurse is skilled at assessing the home situation for an elder living alone and is able to recommend appropriate support and care giving services available in the Upper Valley. The Nurse can be a contact for families who live far from Hartland, assuring them that all is well when it is and alerting them to issues or conditions that need attention. The home visits will be of real support to elders returning to their Hartland homes after a hospitalization or rehab.

One of a growing number of Community Nurses in the Upper Valley, the position is supported by the Upper Valley Community Nursing Project, the town of Hartland, the Ottauquechee Health Foundation and private donations. We welcome your referrals and financial support.
