HARTLAND COMMUNITY NURSES

The Community Nurses are available for home visits for older Hartland residents for assessment, support and referral to appropriate resources. The Community Nurses do not provide direct medical care. 

Each Community Nurse is skilled at assessing your home situation or for an elder living alone. The Nurse will recommend appropriate support and care giving services to enable you to age in place, at home or at a residence of your choice. The Nurse can be a contact for families who live far from Hartland, assuring them that all is well if it is and alerting them to issues or conditions that need attention. The home visits will be of continuing support to elders returning to their Hartland homes after a hospitalization or rehab.

One of a growing number of Community Nurses in the Upper Valley, our program is supported by the Upper Valley Community Nursing Project, the Town of Hartland, the Ottauquechee Health Foundation and private donations.
