Inferior Vena Cava Filter Thrombosis: A Review of Current Concepts, Evidence, and Approach to Management (Hospital Practice)
The increased risk of venous thrombosis within and below the inferior vena cava (IVC) is the main long-term complication of IVC filter placement. In this article, we discuss current concepts regarding the incidence, risks, and management of IVC filter thrombosis. Evidence of the association of each of the following factors is reviewed: type and design of the filter device, population demographics, underlying hypercoagulable states/anticoagulation, modality used to assess for thrombosis, and length of time elapsed since filter placement. Certain double-basket filter designs and a hypercoagulable state are associated with increased incidence of IVC filter thrombosis. Most cases of IVC filter thrombosis are asymptomatic. While large series data on the use of magnetic resonance imaging for the detection of filter thrombosis remain unavailable, evidence suggests that contrast-enhanced computed tomography is preferable to Doppler sonography. A proposed algorithm for the management of IVC filter thrombosis is provided.

I attended a talk titled ‘Prints for Pleasure’ by Tony and Marjorie Furmston, FRPS, at Saint Helens Camera Club. They suggested a Photoshop processing technique. I was trying it and it did not quite work on this photograph so I did some experimenting and developed a very similar procedure. The photograph is unfinished in that I would like to tone down the headland and town and de-fringe the stonework. But, I will save that effort for a different photograph. A will not bore you with a blow by blow description except to say that the building and foreground were treated differently from the sky. The sky is the interesting bit. The ‘Channel Mixer’ adjustment layer is used, then the ‘Black & White with Red Filter (RGB)’ preset applied. The blending mode ‘Vivid Light’ was used at 89%. A very grey featureless sky was considerably perked. Nikon D3, 50mm f/1·4 G. 1/320s, f/6·3, ISO 200, polarising filter, hand held. -5963

What and where? It was built in 1763 and is the oldest of its type surviving in the UK. Went for a walk on the bank holiday and found this. It was so windy, the only way I could hold the camera steady was to lie down. It was taken in brilliant sunlight and I was conscious of the juxtaposition between the building and the sky. The sky is accentuated with a polarising filter then pushed around in the colour channel. Fuji S5, Nikkor 50mm f/1·4 G. 1/290s, f/5·6, ISO 100, polarising filter.

Unfortunately, anticoagulation frequently fails or is contraindicated. Use of inferior vena cava filters can be an effective alternative in these scenarios. Though inferior vena cava filters have been used for > 4 decades, the evidence behind their use is limited. Use of IVC filters is associated with both minor and major complications. More randomized prospective trials are needed to evaluate these devices. In this article, we review issues concerning the use of inferior vena cava filters.
