Living Well With A Chronic Condition

Living Well With a Chronic Condition

Living Well with a Chronic Condition is a Self-Management Program offered for two and ½ hours, once a week, for six weeks.
People with different chronic health problems attend together.
Workshops are facilitated by two trained leaders, one or both of whom are non-health professionals with a chronic disease themselves.

Chronic conditions may cause you to lose physical conditioning and suffer problems over many years.  Some examples of chronic conditions are:
Arthritis, heart problems, asthma, stroke, pain, cancer, osteoporosis, diabetes, obesity, high blood pressure and emphysema.
In contrast, acute conditions are short lived, and with prompt, effective treatment you can usually return to normal health quickly.
A chronic condition is one that many people must learn to live with for many years.  The Living Well With Chronic Conditions Self Management workshop helps you learn how to manage your symptoms so you can get the most out of life.

Subjects covered include:
Techniques to deal with problems such as frustration, fatigue, pain and isolation.
Appropriate exercise for maintaining and improving strength, flexibility, and endurance.
Appropriate use of medications
Communicating effectively with family, friends, and health professionals
Nutritional importance
How to evaluate new treatments.

The companion book, “Living A Healthy Life With Chronic Conditions”, 2nd edition, is provided for class use, and can be purchased for $20 for those that wish to keep the book.  An audio relaxation tape is provided for each participant, at the end of the 6th session.

What you do about something is largely determined by how you think about it.  Some of the best self managers are people who think of their illness as a path.  This path, like any path, goes up and down.  This workshop will help you learn the skills to travel this path.
Classes are highly participative, where mutual support and success build the participant’s confidence in their ability to manage their health and maintain an active and fulfilling life.

You will learn how to:

Manage your symptoms
Communicate with your doctor
Lessen your frustration
Fight fatigue
Make daily tasks easier
Live a healthier life
Much, much more!
