For Transcripts Request, please fill out the FERPA Release Form
For any records requests other than Transcripts, please send an email to our registrar Jannice Correa at: jannice.correa@simivalleyusd.org
Please include the following information information:
Student's First and Las Name.
Date of birth.
Year of promotion or the year last attended.
The complete name of the person requesting records and relationship with the student.
Current phone number (in case there are any questions related to the transcript request).
Address where you wish the records to be forwarded to.
Note the following:
If the transcripts/records will be personally picked up rather than mailed, please bring a valid form of identification with picture.
