What is FinalForms?

FinalForms for Michigan schools is an online registration and data management service used by many districts to handle required athletic and back-to-school forms for students. It allows parents and students to electronically complete, sign, and submit forms, saving time by storing information from season to season and year to year, so data doesn't need to be re-entered annually. The system centralizes information, provides automated reminders, and can also be used for other school-related enrollment and compliance needs.  

A public school sports physical is generally good for one year. It is recommended to have a new physical each year to ensure a student athlete's health status hasn't changed, and to check with the school and the state's athletic association for any specific local policies.
