We recently had a productive and insightful meeting with the Sheffield Emergency Care Forum (SECF) Patient and Public Involvement (PPI) group, which generated valuable discussion around Same Day Emergency Care (SDEC).
A key question during the meeting was whether SDECs are making access to urgent and emergency care more complex for patients when navigating emergency and urgent care.
Participating in the meeting were our PPI co-applicant, Howard Whiting, our new PPI study lead, Jo Coster, two paramedics from Yorkshire Ambulance Service, a medical student, Professor Steve Goodacre (ED consultant) and five members of the SECF PPI group.
Paramedics in attendance at the meeting clarified that SDECs aren't an option patients choose but rather a service they are referred to. One paramedic shared their experience of a newly opened SDEC that was overwhelmed within a month and subsequently stopped taking referrals, which caused confusion among ambulance staff. Another meeting attendee recounted a personal story about their elderly mother needing to visit an SDEC four times for tests and results. This was recounted as an experience that was traumatic and logistically difficult, prompting questions about whether an inpatient admission would have been more appropriate.
Overall, the group expressed cautious optimism about SDECs, acknowledging their potential but questioning their suitability for all patient groups, especially the elderly or vulnerable.
There was curiosity about the role SDECs will play within the context of the NHS’s upcoming 10-year plan, and whether more community-based SDEC-style services might emerge. We also discussed the need for more research that was evidenced by a review of published studies by the Evidence Synthesis group for NIHR Sheffield (EnSygN).
The discussion concluded with general support for the scope of our research and our planned next steps. Our discussion highlighted both the promise and the practical challenges of implementing SDECs within the broader emergency and urgent care system.
We will continue our plan of action and actively engage stakeholders at key milestones throughout the study.
