Please complete a liability form for EACH child. For those who do not have access to a printer, there will be copies in the office.

4. Payment

The camp fee is $280 per camper.

1 child: $280

2 children (from the same household): $560

3 children (from the same household): $840

4 children (from the same household): $1120

** Please complete payment before May 4th, 2025.**

5. *Please bring a hard copy of 'Liability Form' with WET SIGNATURE on the first day of camp OR drop it off at the office.

*A signed liability form is required for participation. If SFVJACC does not receive a completed form by the first day of camp, your child will not be able to participate.
