The following forms can be assigned to your patient within the patient chart:

Authorization of Session Participation-- patient completes
Caregiver's Authorization Affidavit-- patient completes
Conflict of Interest Disclosure-- patient completes
Release of Information-- patient and provider completes

How To Assign a Form To Your Patient
To utilize one of these forms within SC 2.0, go to the patient record and click "Assign a Form".
Use the dropdown menu to select the desired form, choose a due date (if applicable), then click "Save Assignment". This will then send your patient for completion.
