When a client graduates from treatment, needs higher level of care, or discontinues therapy services they can no longer be seen here. To document a discharge, you will add the following template to the "Plan" section of your note:

***Note: If transfer/discharge was initiated between sessions and you are unable to document in your SOAP note, document the above blurb by selecting "Add a Patient Note"
If the client has Kaiser, in addition to the above, you must complete a Discharge Summary in Lucet under "Notes".
No other discharge paperwork is needed unless your client specifically requests it.
When Changing Case Status to Inactive or Discharged
It is your responsibility to mark your clients as inactive or discharged.
Inactive -- no longer your current patient, but patient is still seeing another provider at Sensible Care
Discharged -- no longer a patient at Sensible Care
To change the case status:
Click on "Cases" > select the case you would like to change the status of > click the "Status" dropdown > select appropriate status
When you mark a client as inactive or discharged, you will be prompted to select a "Reason" from a drop down menu.
Please select the best that applies.
You will then have the option to "Delete upcoming appointments from provider calendar".
This will remove all future scheduled appointments for this client from your calendar, rather than deleting each appointment one-by-one.
