Adding Diagnoses
Note: Do not delete diagnoses unless entered in error.
How To Add Diagnoses To Patient Record
Go to the patient's chart
Scroll down to "Diagnoses"
Click "+" to add a new diagnosis
Select "Diagnosis Code"
Ensure "Start Date" matches the date of session where diagnosis was new
Indicate if diagnosis is "Acute" or "Chronic"
Click "Save Diagnosis"
How To Add Diagnoses To Note
1. Once you have completed the above steps, and you see the diagnosis is saved on the patient's chart under the "Diagnoses" section, you are able to add a diagnosis to your note
2. Go to your note, under the "Progress Notes & Patient Notes" section
3. Under "Diagnoses", click "Select option"
4. Any diagnosis that has been added to the patient's chart will be available for selection
Note: If your patient is seeing other providers at Sensible Care, there may be additional diagnosis codes available for selection; only select the codes you have diagnosed
5. Select applicable diagnoses
6. After completing this process for your initial note, previous diagnoses will carry over into your following notes if you select "Import Previous Note" from the cogwheel
How To Mark Diagnoses Inactive
1. Go to the patient's chart
2. Scroll down to "Diagnoses"
3. Click "v" next to the diagnosis you would like to change the status of
4. Click "Edit Diagnosis"
5. Under "Status", change from "Active" to "Inactive"
6. Click "Save Diagnosis"
7. In your next note, only select the diagnoses that now apply
