How to Transfer a Client to Another SC Therapist
When a clinician has identified a transfer therapist you will contact the therapist directly to facilitate transfer of care. Contact Clinical Resource Manager if you are unable to find appropriate transfer.
Transferring Clinician:
When you have identified a transfer clinician within Sensible Care, coordinate with accepting clinician and client for transfer.
Complete "Discharge Summary" in Lucet
Provide MRN to accepting clinician
Mark case as "inactive" in 2.0, and "hide from view" in patient file of Lucet
Accepting Clinician:
Coordinate with transferring therapist and client to schedule.
Add client to "cases" on Sensible Care 2.0 and schedule appointment.
On day of appointment use MRN on Lucet homepage, select "Add Assessment" to send TPI and add to patient list.
Complete "Freeform Note" on Lucet to indicate continuation of care.
Document in Sensible Care 2.0 for follow up note.
