Ensure ALL patients are assessed for suicide/safety. Know their risks and their identified supports including an up-to-date emergency contact name and number.
Inform patients in the first session that if there are any concerns around their safety and they either miss appointments and/or are unreachable, that their emergency contact/identified support will be contacted.
If no contact continues and/or the emergency contact/support cannot be reached nor verify safety, inform that the police will be contacted to complete a well-check.
Document all attempts of contacting/communication.
Remind patients that their safety is important and Sensible Care takes it very seriously.
NOTE - if there is no contact and no concern for the patient’s safety, after 3 documented communication attempts, proceed with discharging the patient.
