We have heard your comments and concerns and have made the adjustments that you've requested.
The medicaid form is now available to be completed every day.
In order to make this accommodation, we needed to change to submitting only one student at a time.
If you work with more than one student, you'll need to submit the form more than one time... three students = three submissions.
In order to complete the online form - each day, sign onto the intranet and navigate to the Special Education page. You'll get to it from the Ed Services Directory:
The process is the same, except it will now take you to the daily form!
Then, scroll down to Teaching Assistant Resources:
Finally Select Medicaid Billing
Then Complete the Google Form Online.
*NOTE: Once you are on the Google Form, you can bookmark it so that you can just come back to that form without needing to navigate through the Intranet.
Remember when filling out the form at the end of the day:
For your name, fill out your complete first and last name.
You MUST include the FULL first and last name of the student (not last name initial, must be the full name)

No. Not after Monday, September, 28th we'll switched to the Daily form.
Yes. Just remember that on the daily form, you're only allowed to enter information for ONE student at a time.
No one. That is to say, unless your building administrator or program administrator wants it.
It's up to you. Just remember that you can only submit for one student at a time, one day at a time. So if you work with 5 students, you need to do the log 5 times (it's not too bad though - the only thing you're typing multiple times is your name).

Over the past few years, the amount of money that has been paid to CUSD 308 from Medicaid has been steadily increasing. This video takes paraprofessionals through the process for completing the Medicaid billing procedures.
Please be sure to review for these inaccuracies before sending them for entry into billing software.

You can use either of the two forms here to help keep track of your students throughout the week until Friday (or Thursday) when you submit the Online Form. You DO NOT need to keep or turn in the hard copy.
