Online Clearance Portion

Online clearance must be completed for each sport you intend to participate in this year.

Step 2

Complete the Log-in Process.

Step 3

Complete the Online Forms.

Step 4

Complete the Online Signature Agreements. *

* - If purchasing Student Insurance, please turn in a copy of the printed ID card to the Athletic Training Room

Physical Form Portion

Physicals are required annually and must be valid throughout the entire season of your sport.

Step 1

Step 2

Complete the top portion prior to doctor visit. If you cannot print or have a physical on a different form, that is ok.  We will accept athletic physicals on the doctor's office form.

Step 3

Have a licensed medical doctor (M.D. or D.O.) complete, sign and stamp the bottom portion of the form.

Step 4

Upload a copy of current physical to the athleticclearance.net website or bring it directly to the Athletic Training Room.
